TO PRINT & FAX YOUR FORM:

1. Right click (Ctrl+Click on a Mac) on the form link above and choose "OPEN" to print.
2. Complete the form and fax it to 888-298-3359.

TO COMPLETE YOUR FORM ONLINE AND EMAIL:

1. Right click (Ctrl+Click on a Mac) on the form link above and choose "SAVE AS" to save it to your hard drive with any name that you desire.
2. Open the file. Tab between the fields to fill in the form.
3. Save the file again.
4. Compose an email indicating that you are submitting your Visionary Circle Enrollment Form.
5. Attach your completed form to the email and send.

Tips to ensure that you begin to receive valuable communications quickly:

1. Please include all of the requested information on your form, especially a VALID EMAIL ADDRESS.
2. A detailed email confirmation will be forwarded to you within 2 business days of receipt of your form by fax or email.
3. To ensure your email provider does not reject our communications:
1. please add visionarycircle@soulpurpose.com to your address book
2. make sure you have not surpassed your storage capacity for your account. Delete older emails which are not needed from your inbox and be sure to EMPTY YOUR TRASH (otherwise it still uses storage space).
4. If you need to update your information...include a sponsor, etc...please note on the form or in the body of your email that this is "RESUBMISSION".
